After you schedule your first appointment, our team will provide you with a link to our new patient forms, which you can also access by clicking on the link below. By filling them out online, you can save time and provide our team with your insurance information beforehand, allowing us to create a more seamless experience for you.
Dental Insurance
Our office is in-network with most major PPO dental insurance plans, and we accept most PPO dental insurances. This means that we’re happy to file claims on your behalf to ensure your benefits are maximized at every appointment, regardless of whether we’re listed as “in-network” with your insurance company or not! Our team will also conduct complimentary benefits checks before your treatments so we can give you a more precise estimate of any related out-of-pocket expenses. This will help keep you from receiving any surprise bills in the mail, which can be a common occurrence with corporate practices!
In-House Membership Plan
If you’re uninsured, we would love for you to join our in-house membership plan, which makes keeping your oral health on the right track simple. For an annual fee, you’ll receive coverage for all of your routine preventive dental services, including two checkups and cleanings and necessary X-rays, as well as a discount on other services you require!
